Items you will need to submit:

       The length and width of the gazebo
       The height of the gazebo
       A [site plan] or [survey] that has the location of the gazebo drawn on it
       Plans, designs or brochure pictures of the gazebo.
       What materials the gazebo will be made of
 

Double check the following:

       The gazebo must be outside of any side or rear [setbacks]
       The gazebo must be outside of any [utility easements]
